Select menu Create-)Reference-)Indexes and Tables... in Index/Table tab

Indexes and Tables dialog pops up

Input a title, select User-Defined from Type combo box

Create from section is changed

An Object list is shown

Check on some options from Create from section, OK

User-Defined table is inserted, just like:

(Title)

(Object name)......Page number

(Title) : The string you input in step 4

(Object name) : Object name

Do step 4,5 to insert another User-Defined Tables

Check on/off some options in the object list, OK
