I have another document that was created in an unknown version of MS Word and
which opens perfectly in Word 97, but OOo 1.1 doesn't handle it very well.

I will send a copy of the document with this report.

Checking the document attached the only thing wrong seems to be that
the date "tick boxes" overlap the Quarter (please tick) heading.  Are
you seeing anything else apart from that?  If so, perhaps you can
attach a screen shot of the offending area?

Yes, can see. This is the same as issue 11493. Writer was not designed
to display frames above the top page border. Will be solved in OO 2.0.
