How do you remove a Reference without deleting the text?
Attached is a document with a heading that is also a reference.
The reference was created by selecting the text, copying the text, opening the
Field Definition dialog (control-F2), Selecting References, Set Reference, paste
the text into Name, and clicking Insert.
A comment in bug 60792 says

Right mouse click on the entry: "Index Entry"
- travel between the entries using the < > buttons
- delete the entry you want

In version 2.1, there's no "Index Entry" in the menu that appears for a
right-mouse click on the text containing the Reference definition.

The Navigator shows the Reference, but there's no obvious way to delete the
reference.
