Using OOo 1.9m122 with a big document (6MB, 100 pages) :
- title
- contents
- introduction
- chapters (1,2,3...7)
- conclusion
- lexical index

When using 'update lexical index', words wich match in the contents table are 
put in the list. IMHO, its a bug or a concept error.
At least, the lexical index update box should gives the choice to include 
automatic index or start page number...

There is no way to know if counting an index entry makes sense or not. The
concordance file simply retrieves all occurences of the searched words.
To avoid the words of the TOC to be counted with, update the TOC first and then
the index.
