Add "Chapter no. without separator" to the Insert Index/Table dialog

This is a request to include a "Chapter no. without separator" button to the Insert Index/Table dialog. This will allow chapter numbers such as A-1 when the Chapter (for example a Heading 1) has a numbering of Appendix A, with the "Appendix" being part of the separator before the number. This will make creating Table of Contents easier in large documents without resorting to manually editing a Table of Contents. This is similar to what occurs when you can insert a field in a footer that either has a Chapter no. or a Chapter no. without separator.

After thinking about it, and some testing, I believe Issue 14725 would be far more useful than this feature enhancement. This is because this issue would display the paragraph number in front of the page number which would not match the page number of the reference. Example: In the body of a document, there would be on page A-1: A Heading 1 text A.1 Heading 2 text However, the TOC would display: A Heading 1 text...........................A-1 A.1 Heading 2 text.......................A.1-1 The above could be a problem. So I wouldn't be suprised if this Issue is closed as WONTFIX as it wouldn't be too useful for page numbers.

The patch I'm going to attach can be used to derive an OOo version able to obtain the document proposed in #desc9 above. It should not be used for a product OOo, since it can provide a mean to generate the index body, not a new index source (e.g. the index generation data is not saved in the ODT document, whereas the index body is). See ML thread indicated in #desc8 above.

-> mru. I'm going to attach two files. The first is a Writer test document I used to check the feature, and the second is a bare OOo extension containing the Basic macros used to generate the index for testing. Both could be used to derive some stuff for QA, I think.

I cannot find the new button in DEV300_m2 and not in DEV300_m8
OK, the additional button is not there because the first attachment is outdated, but no one told us. The Spec Doc http://specs.openoffice.org/writer/index/index_and_tables_dialog_enhancement.odt states that an additional Levels spinbox exists, which I did not see at first try because the Entries tab page UI changes according to which element got activated in the "consruction line" of that tab page. BTW, I only could find the Spec Doc by reading the feature announcement. The Spec Doc is currently not linked from specs.openoffice.org.
