page 75 - notice numbering of title items. automatic numbering should start at 
1, instead it starts at 2.

Yes, I can see the problem... but when I try to strip the document down, the
numbering in the table works fine... I take more investigations on it...

MRU->FLR: I am not able to strip down this document; you'll get the confidential
document via mail. Please delete after usage.
When I opened the doc on my machine, the error was to be found on page 77.
There, in the left column of the table, the numbering should start with 1.
