(The problem occurs also with Windows XP)
We have in the OO configuration the standard fileformat for writer documents 
set to "Word 97/2000/XP". (Because mostly we want to save in Word Format for 
other users.)
Now if you make the following steps:
1. Create a dbase DB (a simple one is attched as zip file)
2. Insert it as Datasource
3. Run the autopilot form
4. Step through it, the last step is to save the document (there correctly is 
only sxw selectable)
5. Open the saved document
Now there are all formfields missing. (There are only borders.)

There are two work arrounds for us at the moment:
1. Do use Openoffice fileformat as default (which we can't)
2. Stop the autopilot with cancel before finish. Turn off the edit mode and 
save the document by ourself.

(There seem to be some issue how the autopilot saves the document.)
