The form letter function appears to function normally, with database displayed 
and a particular record selected.  With output to a file enabled, pressing the 
browse for path button reliably causes an immediate crash.  When the path is 
typed in, the process goes through the motions of completing, but no output 
file is found.

My appologies if this submission is not properly completed.
