Every icon should have only the same one name, no matter in which application,
toolbar and dialog it appears.
For example, the icon with the tip name "Find & Replace" has the name "Find
On/Off" in the Customize Toolbars - Add Commands dialog.
The "What's this?" icon in a toolbar has the name "Extended Help" in the dialog.
This makes it very difficult to tell a user which icon to click. In the last
example, even a picture doesn't help because several commands share the same icon.
The Add Commands list needs a lot of improvements. Even the text in the dialog
is wrong: you cannot drag an icon directly to a toolbar in 680m55
