Exporting HTML or Writer document to PDF excludes form controls.
Tried in Windows 98 (OO1.1.2, OO1.1.1, OO1.1.0) and SUSE 9.0 (OO1.1.1)
Tools->Options->HTML Document->Print->Controls was enabled.
I managed to export the form controls once in Win98 & OO1.1.2 and once in Win 98
& OO1.1.1 by disabling Tools->Options->HTML Document->Print->Controls and then
enabling it again. This worked only once and in SUSE did not work at all.

ooo 2.0 will have form controls, have a look at i31197 or i20327
