Removing optional XML declaration of XHTML output (ie. "<?xml version="1.0"
encoding="UTF-8"?>").
This is helpful as the original OOo binary HTML import filter is not able to
work with the declaration and shows it as text output.

Only CWS handling and check in with counter test is missing.

The allowed removal of the declaration, results into problems of character
encoding for Mozilla 1.6 => Won't fix
