Insert (Menu) -> Indexes and Tables (Menu) -> Index and Tables (Menu) -> Entries
(Tab).  I totally missed that the middle part of the form depends on what
structural element you select.  There is no visual clues.  And it does not
change for E# and E, but has crucial presentational information (for me) when
you click on T.

how to make this dialog better?

Perhaps adding '...' to the button, or better convert it to a tabbed menu if all
the information can on be on the screen at once.

Please see issue 91231 in which I give some suggestions for a more intuitive
dialog. The issue was closed summarily, but I think I gave some very helpful
comments. I would really like to see usability improved.

To grep the issues easier via "requirements" I put the issues currently lying on
my owner to the owner "requirements".
