We cannot currently correctly export hyperlinks to outlines in a way that word
can use. e.g. issue 3256

Have to have a think about this, we'll have to detect the special
hyperlinks and add bookmarks to the text being referenced and replace
the link with a link to that bookmark instead :-(

Set a 2.0 target as it impacts on toc issues.

cmc->mmaher: Writer can have hyperlinks to headings, but there are no actual
"bookmarks" in the heading entries, it all works magically with implicit links.

In word when you do this it creates explicit bookmarks for you in the headings
to which you have linked. For export to work correctly we need to do the same
thing. There is similar code in the html exporter which shows the exporter
getting the list of things which will be linked to and require explicit
bookmarks to be put in them to make it work correctly in a browser.

MRU->MMAHER: fix didn't make its way into CWS build. Links in index are not
usable in Word after export.
