1st page only contain Please complete the following: - , but no form
at all.
the form is available in 1st page when it is opening using MS Office XP

using OpenOffice 1.1 (default Install, US), Win XP Pro Sp1. 
(And MS Office XP Sp2).

The form is inside a table cell, which is larger thn one page. Thus it
is moved on second page and some controls are missing in the end. See
issue 2109 for this.
