This problem happened in 638c.

1. Activate Writer.
2. Select "File" and in the file history list, pick up a file which is no
longer exist.
3. OO reported an "An unrecoverable error" has occurred.

638H_7138: I choose a file (which was deleted) from the file menu
picklist.
I get a message about "Error loading document file...; Nonexistend
object; Nonexistend file. I can close this message without any
problems.

I could not simulate the problem again in another PC.

I had removed all previous installation of OO and deleted the
directory and reinstalled 638c again. The same problem didn't happen.
So I would suggest closing this problem.
