When I follow the instructions below to start the page numbering in a Writer 
document at a number greater than 1, it works if the first page in the document 
has the default page style. However, if the first page has any other page 
style, the page number remains at 1. I found this effect in 1.0.1, 1.0.2, and 
1.1beta. I do not have any of the developer editions to test them.

Here are the instructions I received, from Solveig Haugland, expanded by Andrew 
Pitonyak.

* Create a new document
* Choose Insert->Footer->Default

The cursor will now be in the footer so add the page number.

* Choose Insert->Fields->Page Numbers

* Move your cursor to text. This is empty because this is a new file.
* Type the text "Top Of File".
* Choose Format > Styles > Catalog (or Ctrl+Y).
* Choose Paragraph styles in the top drop down box.
* Choose "Heading 1" in the list box.
* Click on the New button.
* Set the name to "Page Test".
Linked With should say "Heading 1" and  Category should say "Text Styles" at
this point.
* Click on the "Text Flow" tab.
* In the options section, place a check next to "Break".
* In the options section, place a check next to "With Page Style".
The Page Number section should now be visible.
* Enter page 45 in the Page number section.
* Choose Ok to close the dialog.
* Choose Ok to close the other dialog.
* Select the text "Top Of File".
* Choose Format > Styles > Catalog (or Ctrl+Y).
* Choose Paragraph styles in the top drop down box.
* Choose "Page Test" in the list box. 
* Click the Ok button.

Your page number should be 45.

I have now discovered that if I follow the procedure using the 
Default page style for the first page of the document, I can change 
the starting page number (by changing the number specified in the 
first paragraph style). I can also change the page style to something 
else, and the starting page number stays at the number I specified; 
however in that case, I cannot change the starting page number to any 
other number. Also, once I change the page style to anything other 
than default, then even if I change it back to default, I can no 
longer change the starting page number. This sounds very much like a 
bug to me. It exists in v.1.0.3 and in beta2.

I cannot duplicate this behavior on the linux version of RC3.  What I
get is First Page style (you have to change the page style to see
something different).  However, "First Page Style" is not set as a
Break With Page Style in "Footer" so it should not make the new page
style have a number of 45.  The only way to change it to something
else is to redefine the Footer paragraph style break to apply to First
Page style (on my test).

As I understand how it works, what we now have is a footer style that
will create a default page NUMBER but not the associated page STYLE. 
I'm not sure what the expected behavior as it would never have
occurred to me to set a page break with the default footer.  If I were
to use the footer, I would automatically set a custom footer,
otherwise you get a new page with every footer and get really strange
results if you want any page style other than that for the footer.

What would I expect?

For one thing, if I changed the page style with a footer style, I
would expect the default footer to force a default page style no
matter what I attempted to set -- in effect, forcing me to have a
unique footer style for each page style.

However, and this appears to be the bug, if I choose a new page style,
using the method suggested above, I create an unresolvable conflict as
I am using only one footer style.  The problem is fixable (from a
user's point of view) ONLY if there were a footer style option on the
Footer tab in the page style dialog AND a warning if you use a footer
style that is bound to a page style already.

However, OOo does not now and never has had any ability to use custom
footers without a lot of extra work.  So perhaps this whole thing can
be subsumed in an enhancement request that the header and footer
styles (and frame styles, for that matter) be selectable, thus making
custom headers, footers (and frames) useable instead of just pretty
decoration for Writer.

Please try using the latest OpenOffice 1.1 Final , you can download it
from www.openoffice.org
many bug fixes and enhancements since your version and 1.1 Final . If
the problem still happend in 1.1 please report back 
It maybe not yet solve your Issue, but it worth to try.

The problem I reported on 2003-06-24 still exists in 1.1 final. 
If I am using the default page style, I can make the starting page 
number greater than 1 (by changing the number specified in the first 
paragraph style). But if I am using any other page style (such as 
First Page), then I cannot set a starting page number greater than 1. 

After I have set a page number using the default page style, I can 
change the page style to something else, and the starting page number 
stays at the number I specified; however in that case, I cannot change 
the starting page number to any other number. 

Also, after I change the page style to anything other than default, 
then even if I change it back to default, I can no longer change the 
starting page number.

I can confirm this. Will attach two files for demonstration:

First doc created with steps given in the first comment, starting with
default page style. Change page style to First Page style, "45" remains.

Second doc created with the same steps, but started with First Page
style. Page number doesn´t change to "45".

You can still set the new page number as a hard formatting of the very
first paragraph (edit the paragraph, not the paragraph-style!).
When applying the custom-heading style, the page number is not set to
45 as defined in the style, but set to 0 (=no correction). Setting
this to 45 manually will give you page number 45 with the "first page"
page style.

Thanks, Christian -- that works perfectly, and is much more logical 
than using a paragraph style. Obviously the instructions were wrong. 
I think this problem is now FIXED.
