The document's XFunctionProvider needs to be created as part of the document load.
This issue is a collation of issues:
11718 - warning dialog not launched when  run macro set to always and warning
checkbox active
11727 - warning dialog does not get launched when opening document
11779 - warning dialog does not get launched when  confirm in case of other
document sources is set on
