Followup from sf.net bug 643045.

The date format for INPUT on czech locale is NN, D. MMMM YYYY which is unusable.

Also the table of available formats is filled with duplicate formats and missing
some important ones. For the correct values I suggest to use the slovak locale.

The locales are from version 1.1 beta 2 up made in another way. There
are still some duplicates, but the format is match more usable.
