Steps to reproduce:
1.  Create form under 643C or earlier
2.  Open under 644 and then save it under 644

Actual result:  
All drop down Combo boxs are missing but OOo still marks field area for them,
formatted fields disappear from table, other text boxes are gone (see Saved
Under 644 screenshot)

Expected result:
Form opens normally (see First Opened screenshot but note corruption in Combo
Box labelled Type -top left-)

damn, bad timing, this bug was in the tree for only a very short time,
but it seems that it made it into the public OOo build :(

already fixed, internal bug id is #106743#
