When I insert a line in a header or footer, I can see the line on the screen and
also in Print Preview, but it only prints the header/footer lines in the first
page, and they are not printed on subsequent pages.

I will enclose a demonstration file with this report.

Duplicate to issue #19919, which is already fixed in internal cws
sw7pp1b. Thus, marking as duplicate.
