To reproduce:

Open a Master Document with several files.
Open Navigator with the toggle on the file list.
Click on one and Insert > File and choose a new file.
The new file appears in the file list.
Click on Toggle to see the headings.
The new file chapter does not appear in the list of headings.

INVALID.  My mistake.  The added file had a different kind of heading.
 Sorry for the wasted bandwidth.
