In last step of Form wizard when you finished form creation, with option work
with the form, form window closes and not opens back OOo crash, form not saves
into Form area - Form lost, but form saving process starts, saving progress can
be seen until OOo crash happens.

opensuse 10.3 i586, 
Crash reports: rvpfwvc and r5pfwvc.

I installed m49 on ubuntu 8.10, and same crashes as in opensuse.
In winXP no crash, but form not saved to database document, but can be found in
temp directory. If this file opened in OOo 3.1, the form connected to database
and data can be edited, if OOoDev closed. I created database file in OOoDEVm49
as registered, and added some data before created form.

Ah, loading those files in a non-product versions shows what's going wrong here.
Indeed this issue is a duplicate of the recently discovered issue 102436, which
is fixed in CWS dba32b (which hopefully will make it into m50, or m51).

Thanks a lot for submitting the issue, even though we found out it's a
duplicate, good to know that will be fixed quickly!
